Case Manager
Job Description
We're seeking a Case Manager for a Berkshire based affordable housing authority. The Case Manager is responsible for screening households seeking services, processing financial assistance applications, and managing stabilization cases to support self-sufficiency.
Key Responsibilities:
- Conduct initial screenings of customers, inform them of available resources, and determine eligibility.
- Process financial assistance applications to completion, maintaining accurate electronic records while ensuring confidentiality.
- Track customer cases from opening to closing and fulfill data requirements for financial programs.
- Provide support and guidance to applicants seeking RAFT financial assistance.
- Prepare reports and perform data entry as required.
Skills and Qualifications:
- High school diploma or GED; Associate’s degree preferred.
- Previous experience in social services or case management.
- Strong time management, organizational, and communication skills.
- Proficient in Microsoft Word, Excel, and Outlook; able to learn new software platforms.
- Sensitivity to the needs of homeless and low-income populations.
- Team-oriented with a mission-driven approach, seeking creative solutions in a diverse, positive environment.
