Case Manager
Job Description
Options counseling can be provided by phone, in the home of the consumer, or in a hospital or skilled nursing facility. This person also plays a major role in marketing and external relationships, maintaining an active presence in the Aging Disability Resource Consortia.
Essential Functions: Work closely with other staff, community partners, and state designated staff to determine consumer’s eligibility for various programs; regularly interact with consumers, their families, and caregivers; understand and be able to explain how to apply for and utilize MA benefits; obtain SHINE certification; entering required documentation correctly and timely in both electronic and written formats; explain benefits systems in a way that consumers can understand; conduct outreach visits throughout Berkshire county to advertise this Independent Living agency.Â
This position is full-time 40 hours (1 hour paid lunch daily) and 75% of work takes place in the Pittsfield office or in the community/nursing homes. This position requires travel with personal vehicle, home visits (ascending/descending stairs), electronic and written documentation, and ability to understand benefit rules and regulations. Primary working hours are 9am-5pm M-F.
Pay range is $38-39.5K annually
