Job Description

If your heart is in Human Services - this may be the position for you! We have an opportunity for an Options Counselor Case Manager to work with a Berkshire County non profit. The Options Counselor Case Manager will educate older adults and persons with disabilities to understand the type and range of community-based long term care supports available to them. The counselor will also provide information and assistance with referrals for community-based long term care benefits.

Options counseling can be provided by phone, in the home of the consumer, or in a hospital or skilled nursing facility. This person also plays a major role in marketing and external relationships, maintaining an active presence in the Aging Disability Resource Consortia.

Essential Functions: Work closely with other staff, community partners, and state designated staff to determine consumer’s eligibility for various programs; regularly interact with consumers, their families, and caregivers; understand and be able to explain how to apply for and utilize MA benefits; obtain SHINE certification;  entering required documentation correctly and timely in  both electronic and written formats; explain benefits systems in a way that consumers can understand; conduct outreach visits throughout Berkshire county to advertise this Independent Living agency. 

This position is full-time 40 hours (1 hour paid lunch daily) and 75% of work takes place in the Pittsfield office or in the community/nursing homes. This position requires travel with personal vehicle, home visits (ascending/descending stairs), electronic and written documentation, and ability to understand benefit rules and regulations. Primary working hours are 9am-5pm M-F.
Pay range is $38-39.5K annually
